We offer support (to 18+ patients) with well-being concerns to anyone registered with a Stockport GP ranging from anxiety and low mood, finance and housing support to loneliness and long-term condition management.
We can help patients access local support services and groups within the community and offer guidance on what local resources are available to patients.
The service supports helping patients to make positive life changes to improve health and well-being.
