The Cancer Care Coordinator will be a single point of contact to holistically support patients with a cancer diagnosis within the community. The coordinator will work with patients to help with any issues or worries that may arise around their care. 

How we can support patients:

• Completing 3 and 12 month cancer care reviews 
• Signpost to services within the community e.g., mental health/counselling services, benefit/financial services, alternative therapies 
• Liaise and refer to social prescribing team 
• Supporting conversations about end-of-life care if appropriate 
• Support around cancer screening programmes and appointments
