Referrals can be made by EMIS task, phone or via email.

I offer assessments and reviews for complex mental health cases and provide clinical leadership on mental health. 

I work with the GP mental health lead, mental health care coordinators, Trainee Wellbeing Practitioner and Social Prescribers to ensure patients' needs are met to maintain optimum mental health and wellbeing.

I offer assessment and follow up appointments to determine the best plan of care for each individual.

I review or support GPs to review (as appropriate to individual patient) to optimize mental health care of patient (biopsychosocial aspects) on SMI register who are not currently under Secondary care (Pennine Care) and work to identify hard to reach patients who have not attended for annual SMI mental/physical health review and adopt assertive outreach approach to establish needs/engagement.
